  • Hold a valid work permit. 
  • Engage in full-time employment for an Ontario-based employer for at least 6 months. 
  • Maintain your primary place of residence within Ontario. 
  • Be physically present in Ontario for a minimum of 153 days within any 12-month period. 
  • Be physically present in Ontario for at least 153 of the first 183 days immediately after establishing residency within the province. 

Coverage Offered by OHIP – 

It is essential to note that, to be eligible for a service, you must have a valid medical reason to receive the treatment. OHIP offers health benefits such as: 

  • Appointments with your family doctor. 
  • Visits to walk-in clinics and select healthcare providers. 
  • Emergency room visits for urgent medical needs. 
  • Medical tests and essential surgical procedures. 

How to Apply for OHIP 

Applying for OHIP is a simple process that requires an in-person visit to a Service Ontario center.  

  • To complete your application, you will need to provide the following documents: 
  • A completed Registration for Ontario Health Insurance Coverage form (014-0265-82). 
  • Three separate documents from the List of qualifying identification documents: 
  • One original document proving Canadian citizenship or OHIP-eligible immigration status. 
  • One document (original, printed, or digital/electronic) verifying residency in Ontario. 
  • One original document confirming your identity.
